I have completed and met both the HESI A2 Exam and the LPN-RN Entrance Exam requirements and agree to submit the results of both tests via email for the nursing application before the published deadline (further instructions will be provided upon submitting this application).*

Students may be currently enrolled in pre-requisite courses at the time of application. All pre-requisite courses must have a letter grade of C or better and the pre-requisite GPA must be at least 2.5. When students are enrolled in any of the pre-requisite courses during the application period, they will only be eligible for conditional acceptance. Transcripts showing the passing grade need to be turned in to the Health Professions Office upon completion of the course/semester.*

I acknowledge that the theory portion of most courses are delivered in the on-campus classroom environment facilitated by faculty and that there are additional online requirements.*
I acknowledge that I will be required to have access to a computer with reliable internet and that I will be required to have specific technology devices/requirements and technology learning tools specific for each course.*
I acknowledge that the clinical components/experiences for each course are face to face and are dependent on the availability of clinical sites/locations. I further acknowledge that these clinical components/experiences may be held during day, evening, weekday, or weekend hours, and possibly night shifts.*
I acknowledge that working over 20 hours per week puts me at risk for not being successful in the Nursing Program.*
I acknowledge that a criminal background check and drug screening are required for admission to the Nursing Program as required by the clinical affiliates. If I have a criminal conviction I should contact the Virginia Board of Nursing to determine if my conviction will prevent me from obtaining a license.*
I acknowledge that the Nursing Program is committed to the policies set forth by Laurel Ridge regarding disabilities and reasonable accommodations. If I require special services or accommodations it is my responsibility to contact the Laurel Ridge disability support services counselor on either campus for an appointment at least 4 weeks prior to the beginning of classes if accepted into the Nursing Program.*
I acknowledge that I must be able to meet the essential functional abilities as stipulated in the Nursing Student Handbook*
I acknowledge that the Laurel Ridge Nursing Student Handbook outlines the following policy. In all NSG courses students must achieve a minimum of an 80% for their test average for each course in addition to achieving, an 80% for their overall course grade. If a student has a “C” or better in the course but does not have a “C” or higher test average, then the test average is the final grade for the course.*
Clinical experiences may occur at any approved clinical site, typically within 50 miles of my assigned campus. I understand that travel will be required for these experiences. Additionally, travel between campuses may be required for lab, simulation, or lecture.*
